[NCSG-PC] Final cleaned up document for EPDP Phase 2 comments

OK folks, I have cleaned up the document, accepted all but one of 
Milton's comments, written an annex with proposed text for section 13, 
and attach it here in word and PDF.  I presume Tomslin is going to send 
it in on the 30th, absent further changes.  Let me know if I need to do 
anything further.
