[EC-NCSG] working online membership form for NCSG membership applications

Dear NCSG-EC Members:

Good news!  We now have a working online form for accepting membership applications to join NCSG.  Please have a look at:
   https://community.icann.org/display/gnsononcomstake/Membership+Application 


At the end of the form is an option for the applicant's info to be forwarded on to a constituency(ies) for additional membership consideration upon NCSG approval.  

The form is ready to go live now, but please still let me know if you have any feedback on improving it.
