I agree with your sentiments, Joan, as to the purpose of this exercise, but I would just like to add that is not possible to anonymise this data set. In addition to the fields we can see, I suspect the survey platform is collecting IP addresses, the date and time of submission, details on the browser, and other fingerprints which could be used to re-identify the respondent. This is particularly true given the volume of text boxes (versus multiple choice options) present, which allows for one's sentence structure / style to be analysed, and the likely small number of respondents. I think it is misleading for the consultants behind this survey to claim that they will anonymise the responses. They do not have the ability to do so.

It's important that surveys remain anonymous and the focus is on the response and critical input not the collection of personal data.
